This practice has closed permanently.

All health records held at the practice have been moved a secure storage location.

If you have been a patient at the practice, you may request that a copy or written summary of your health record be transferred to you or another practice of your choice.

Completed forms can be emailed, faxed (02 6147 7030) or posted to us (PO Box 7064 Kaleen ACT 2617).

All health records stored at the secure facility will be held until 1 November 2019 when they will be destroyed.
